Billing & Insurance


You will receive bills from several sources for your surgery.
  • Findlay Surgery Center will send you a bill that includes the facility fee for the use of our Surgery Center.
Additionally, you will receive separate bills from the following providers
(if applicable):
  • Your surgeon
  • Your anesthesiologist
  • Pathology, if you use their service (Blanchard Valley Pathology)
  • Pathologist, if you use their service



This is the Findlay Surgery Center Billing Process:
  • Your insurance co-pay is due when you sign-in for surgery.

  • As a courtesy, we will bill your insurance company directly for your procedure.

  • You will receive a bill / statement once all insurance has been applied. You are responsible for any deductible, co-insurance or balance not paid by your insurance company.

  • There are several options available for payment plans though our facility. The acceptable forms of payment are cash, cashier’s check, credit or debit card, Care Credit, or personal check.

  • If you do not have insurance, or if you are having cosmetic surgery, you will be required to pay for your surgery two weeks prior to your procedure, unless other arrangements are made with our business office. A price quote will be made available to you based on the information provided by your surgeon's office. The final bill may be higher or lower based on the actual procedure(s) preformed.

  • Worker's Compensation claims require more information. If your surgery is the result of an accident at work, please tell your doctor's office.
