Pre-Surgical Assessment at Findlay Surgery Center
Thank you for choosing Findlay Surgery Center for your upcoming surgery or procedure. To prepare for your surgery or procedure, you will have a Pre-Surgical Assessment appointment with a nurse from Findlay Surgery Center's Pre-Surgical Assessment department. The nurse will speak with you to gather important information about your medical/surgical history; to identify any tests that you will need done before your surgery; to discuss with you what to expect before, during and after surgery; and how to prepare for your surgery.

To start the pre-surgical assessment process, Findlay Surgery Center requests that you fill out your medical history onlin with One Medical Passport.
